Terms & Conditions:

• IDC Diamond Membership Plan is a 12-month program billed at $29.99 per month, for a total annual commitment of $359.88. By enrolling, the patient agrees to make 12 consecutive monthly payments via automatic credit card billing.
• Start Dates:
• Consultations and exams are available starting Day 1 of membership.
• Cleanings will be scheduled starting Month 3.
• Membership is non-refundable and non-transferable.
• Payment is required monthly via credit card auto-pay. Missed payments may result in suspension of benefits.
• Membership does not replace dental insurance and cannot be combined with insurance benefits.
• Treatments outside of the listed services are offered at a 10% discount for active members.
• The plan automatically renews annually unless canceled in writing 30 days before renewal.
• By enrolling, you agree to these terms and authorize 12 monthly payments to be charged to your credit card on file.
